2007 Letter from the Registrar

Dear Hockey Players and Parents:

We have 94 players who have expressed interest in the upcoming hockey season. Thanks to your response, we have been able to make team projections and schedule ice times. You have done a great job of getting PCYHA off to a great season! Ice time begins mid-October. Look at the website: www.parkcountyhockey.org for more information.

We are again asking players to register online at www.usahockey.com. Players will receive a confirmation page that needs to be printed and provided to me at registration. Registration will be held on Tuesday & Thursday, September 18 & 20 from 5-7 p.m., at the Riley Ice Arena lobby. Please bring in your confirmation number and you will be able to fill out forms and pay fees at this time. The Consent to Treat/Medical History form and the Code of Conduct form are also available on the website, if you would like to complete them ahead of time and bring to registration. You do NOT need your child’s social security number, but you will need a birth certificate if you are a new player, health insurance number, and doctor/dentist phone numbers. The gear swap will be occurring at this same time. If you have gear for the swap, please bring it to Riley Arena the first night of registration.

PCYHA and the Riley Ice Arena are nonprofit entities. The cost of registration does not fully cover ice time, league fees, referee fees, and other costs associated with the hockey program. Therefore, to keep hockey affordable, you will be asked to participate in fundraisers and to volunteer time during tournaments and league play (concession stand, scorebook, scoreboard, sweatshirts). Your team parent will contact you to find out your preferences.

Each participating player must have the following documents on file:
- Individual Member Registration (IMR)/Waiver of Liability
- Signed USA Hockey Consent to Treat
- Medical History
- Signed Code of Conduct for participants and parents
- First time registrants must provide a copy of the player's birth certificate
- All forms must be signed and turned in before the first practice.
Fees for the 2007-08 season are as follows:
• Learn to Skate $30 – ages 3-5 (6 lessons starting Oct. 9)
• Learn to Play Hockey $30 – ages 3-5 (6 lessons starting Jan. 8)

I will be available at the Riley Arena on Thursday, October 18 from 5-7 p.m. to collect the second installment if you choose to pay fees in two equal installments.
